Job summary
The Digital & transformation officer is an exciting new role in the rapidly developing environment of primary and community healthcare management. The successful candidate will play a significant part in the development of PCNs in Easington and Sedgefield supporting the delivery of their Digital and transformational progress.
The purpose of this new post is to provide hands on support to our practices, helping them to understand and maximise the potential of their existing digital technologies and, as appropriate, support and advise on the introduction of new technologies to ensure patients get the best care possible.
NHS experience is not essential, but the ideal candidate will have a natural ability, enthusiasm, and desire to improve the digital and transformational processes within primary care. They will have great problem-solving skills and the patience to explain and support those who arent as digitally minded as them!
Main duties of the job
You will be responsible for a broad spectrum of the digital & transformational work areas as well as supporting data & population health management. Full training & ongoing support will be provided
Actively resolve issues with clinical systems used in practices & across the networks, to limit work disruptions within the services
Improve the adoption of new technology to enhance patient access & experience to support increasing productivity, access, and capacity
Identify best practice & promote the most efficient ways of collaborative working through digital platforms
Standardise, manage & monitor websites & social media accounts
Support practices to increase their patients' participation of digital services, for example online access to ordering medication
Monitor performance & delivery of services against contracts & other national & local performance/reporting requirements
Deliver support to practices against the NHS Digital Journey Planner & ensure practices are conforming with national policy on digital priorities such as patient online access
Support the management team in implementing service improvements identified by digital, public health or other intelligence
Keep up to date with all professional requirements of the Digital & Transformational role/responsibilities as set out by NHS England
Be an advocate for continuous improvement facilitating & sharing good practice & collaborative working across Easington, Sedgefield & the wider County Durham area in all that you do

To support the increased demand on general practice the NHS and government introduced scheme where they provide funding to the Primary Care Networks to employ additional, non-traditional, functions. One of these is the Digital and Transformational role.
