Team Select Home Care supports the efforts of children and young adults who are medically fragile to live at home with their families and thrive in their communities. Our nurses and families have round the clock support from our experienced clinical team. We are currently seeking a Registered Nurse or Licensed Practical Nurse for day, night or weekend shifts to care for a child at their home in Naperville, IL.

Responsibilities

Team Select Home Care is looking for Registered Nurses and Licensed Practical Nurses to join our Team Select Family. Our nurses are responsible for documenting and providing skilled nursing care to our patients, following the developed plan of care and the patient’s physician orders for each of our patients; while adhering to confidentiality and professional boundaries.

Responsibilities include:

• Patient assessments


• Administration of prescribed medication


• Care planning


• Communication with other members of the patient's multi-disciplinary team


• Emergency management

Qualifications

• Current RN or LPN license


• One (1) year nursing experience, a plus


• Pediatric experience preferred


• Experience with tracheostomy, g-tube care, vent preferred but willing to train. New Grads Welcome


• Maintains a current CPR certification
