      What to Expect (Job Responsibilities):
      - Conduct comprehensive pre-admission screenings (PAS) for customers applying for the Arizona Long Term Care Services (ALTCS) program
      - Manage and monitor PAS caseload, schedule interviews, and document follow-up actions
      - Obtain and review medical records and interact with family members, caregivers, and other professionals to evaluate eligibility
      - Communicate assessment outcomes and provide community resources to applicants as needed
      - Attend staff meetings and participate in professional development initiatives

      What is Required (Qualifications):
      - Minimum of 3 years of experience in social service case management, health care services, or relevant fields; or a Bachelor's or Master's degree in a related field with 2 years of experience
      - Valid Arizona Driver License
      - Proficiency in medical terminology and understanding of eligibility requirements for long-term care
      - Ability to communicate effectively, both orally and in writing
      - Familiarity with state and federal rules regarding long-term care services

      How to Stand Out (Preferred Qualifications):
      - Bilingual in Navajo (for Chinle office) or Spanish (for other offices)
      - Experience working with elderly or disabled populations
      - Knowledge of community resources and crisis intervention methods
      - Familiarity with electronic case management systems
      - Strong organizational skills and ability to manage time effectively
