What to Expect (Job Responsibilities):
- Conduct comprehensive pre-admission screenings and assessments in various settings, explaining benefits and eligibility requirements to customers
- Manage and monitor a caseload, ensuring timely completion of assessments and documentation
- Gather and review medical records and relevant data to support eligibility findings
- Interact with other state agencies to coordinate care and services for applicants
- Communicate outcomes of assessments to applicants and provide appropriate community resources

What is Required (Qualifications):
- Minimum of 3 years of experience in social service case management or related fields, or a Bachelor's/Master's degree in a relevant discipline with 2 years of experience
- Knowledge of medical, functional, and psychosocial issues affecting the elderly and disabled populations
- Ability to communicate effectively both orally and in writing
- Proficiency in using a laptop and relevant software for case documentation
- Arizona Driver License

How to Stand Out (Preferred Qualifications):
- Bilingual in Spanish (not required but preferred)
- Fluency in Navajo for specific office locations
- Experience with health care delivery systems and eligibility determination processes
