      What to Expect (Job Responsibilities):
      - Provide skilled Physical Therapy services on an intermittent basis to patients in their homes under the direction of a Registered Physical Therapist
      - Assist in the evaluation and development of a rehabilitation plan of care and participate in the preparation of clinical and daily progress notes
      - Perform routine strengthening and therapeutic exercises and instruct patients and families on exercise programs
      - Document patient activities and findings in clinical records and report changes in patients' conditions to the Physical Therapist and physician
      - Attend rehabilitation department staff meetings and participate in discharge activities with staff, patients, and physicians

      What is Required (Qualifications):
      - Must be licensed or registered in the State of Indiana
      - Completion of health statements acknowledging the ability to perform the duties of the position
      - Valid state driver's license and proof of current automobile insurance
      - Current CPR certification; online CPR certification accepted
      - Graduate of an approved school for Physical Therapy Assistant, with a minimum two-year course

      How to Stand Out (Preferred Qualifications):
      - Knowledge of current federal, state, and agency regulations concerning patient care
      - Experience in patient/family education using various communication techniques
      - Strong computer literacy and ability to maintain simple records in English
      - Participation in agency's mandated activities such as in-services and Quality Assurance
      - Previous experience in home health care settings
