- Plays a critical role in driving success in population health performance by guiding care teams towards best practices, identifying gaps both through data and operational observation, by providing feedback and education, and by guiding actionable change from any gaps and root-causes identified
- Guides and assist practice care teams to integrate population health management tools and programs, including focus on risk adjustment and quality
- Builds trust-based relationships with medical practices and care teams to facilitate change management, engagement, solutioning, and sustainability of population health initiatives
- Delivers and progress reports and review with medical practices and care teams. Identify performance gaps from report data and guide care teams towards solutions
- Provides support for other total medical expense and cost savings initiatives
- Manages the implementation and successful completion of our quality improvement program
- Identifies and prioritizes key quality and utilization initiatives critical to success under performance-based payment programs
- Co-facilitates and/or leads practice manager forums and meetings related to value-based care initiatives
- Provides training and guidance on the use of technology tools to medical practice care teams as necessary
- Stays current with updates to technology tools, and guide care teams in integrating new functionality into workflows
- Assures that standards of practice and policies are in compliance with NCQA and other regulatory guidelines and standards
- Deploys PDSA best practices from one practice to the next to improve operational and quality metrics
How You’ll Help Transform Healthcare:
Serves as the provider relations function for Carilion's Accountable Care Strategies team. Provides education and awarenss of the value based care programs and key concepts such as risk adjustment and quality. Supports improved workflow development to improve utilization of existing population health management technology.
What We Require:
Education: Bachelor’s Degree or equivalent work experience
Experience: 3 to 5 years of experience in practice management and/or provider relations
Other Minimum Qualifications: Experience in population health and value based care; Skilled at data analysis; Experience working within Epic; understanding of Medicare Advantage Star Ratings and risk adjustment.About Carilion
