      Job summary

      The Community Mental Health Services are undergoing widespread, innovativetransformation and investment over the next two years. North London FoundationTrust are at the forefront of this transformation, developing a more holistic, recoveryoriented, biopsychosocial approach to support people with their mental health, inpartnership with primary care, other community NHS providers and the voluntarysector. The aim is to build support systems that reach the whole mental healthpopulation with a focus on prevention and sustained recovery delivered by multiagency collaboration. Each population unit is based on the footprint of Primary CareNetworks, bringing together NHS, voluntary sector, peer support and communityorganisations. These relationships will be central to the new community mentalhealth model.The Population Health Nurse role will support with driving a community andpopulation health approach to the teams work within a locality, forging partnershipsand close relationships with GP's and key local stakeholders. This will support ajoined-up approach to supporting the wellbeing of residents and developing a systemwide approach to prevention.

      Main duties of the job

      The Population Health Nurse role is a senior, specialist clinician position within the South Camden core team. This essential post focuses on addressing complex mental and physical health needs, supporting an integrated primary-secondary care approach, and promoting health equity in the borough. The role involves working with a designated cohort of service users to deliver population health initiatives, foster community connections, and provide leadership in team discussions. With a person-centred, strength-based approach, the nurse ensures timely transitions for service users, manages risk effectively, and supports GPs by alleviating workload pressures. Operationally, the potholder adheres to team procedures and contributes to the delivery of high-quality, coordinated care aligned with clinical strategies.
