- Undertaking health assessments, administering medication and treatments.
- Providing and evaluating a range of health interventions to ensure that patients health and social care needs are met.
- General health and well-being promotion as well as creating individual, person-centred plans for patients with, or at risk of, a long term condition.
- Managing and treating long-term conditions, in line with national and local policies and the practice needs.
- Reviewing medication for therapeutic effectiveness, in accordance with evidence-based practice as well as national and practice protocols; working with patients to support their adherence to prescribed treatments.
- Implementing, and participating in, vaccination and immunisation programmes for both adults and children, including patients travelling abroad.
- Wound care.
- Previous clinical experience.
- Compassionate manner with an empathetic approach and demonstrable kindness in all aspects of your work.
- Knowledge, and experience of its application, in supporting long-term conditions.
- Insight and understanding of current issues in nursing and the wider NHS.
- A relevant nursing degree.
- Current NMC registration with an active PIN.

Job summary
The Lawns Medical Practice located in Diss, are seeking an experienced and dedicated practice nurse to join our team on a full-time basis.We are a supportive and friendly practice with a manageable workload.

We serve the small market town of Diss. We have 7100 patients with a mix of age groups. We actively encourage learning and take a small number of medical students each year. We have an excellent team of staff who support each other.
