- Assess, plan, develop, implement and evaluate programmes to promote health and wellbeing.
- Implement and evaluate individual treatment plans for patients with a known long term condition.
- Proactively identify and manage as appropriate, treatment plans for patients at risk of developing a long term condition.
- Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations.
- Support patients to adopt health promotion strategies that encourage patients to live healthily.
- Provide information and advice on prescribed, or over the counter medication on medication regimens, side effects and interactions.
- Support and manage health needs of women presenting for cervical cytology consultations.
- Recognise, assess and refer patients presenting with mental health needs.

- Evidence of clinical skills and experience in treatment room consultation, phlebotomy, ECG recording, screening, management of long-term conditions, cervical cytology sampling, vaccinations and immunisations, assisting with minor surgery, dressing and wound care, and recording spirometry.

Job summary
Newport Pagnell Medical Centre is looking to welcome a practice nurse on a full or part-time basis into their busy team to provide treatment room duties and ideally be experienced in long term conditions. The hours for this role can be flexible, so please apply to discuss.
These posts will involve working at Newport Pagnell Medical Centre, NPMC@Willen and NPMC@Kingfisher. Flexible working will be considered.
The practice holds Saturday morning clinics 0800-1230 working alongside a duty GP, these are managed by the nursing team on a rota basis and will be additional to the contracted hours. In this role, you will be responsible for the delivery of general practice nursing services to the practice population.

Newport Pagnell Medical Centre is a large GP training practice committed to providing excellent primary health care services with a strong emphasis on staff training and development. Based in excellent premises, our committed teams deliver a wide range of general medical services.
We are a large training practice based in the town of Newport Pagnell with another surgery in the Willen area.
The practice nursing team forms part of our integrated nursing team working alongside our community nursing team, community matrons, urgent care team and specialist nursing team.
