      The Pregnancy Navigator program works with women through their entire pregnancy journey.
      From the initial new OB phone call to postpartum, the pregnancy navigator serves to guide patients and provide resources, information, education, and support.
      The role also includes partnering with Maternal Fetal Medicine, Family Medicine, Women's Community Education, the Family Birth Center, and Women's Leadership to promote optimal patient care and services for women.
      Experience in Women's specialty unit required
      Monday - Friday 8:00a-5:00pm
      No weekends, evenings, or holidays
      Experience is credit to wage

      Integrates health care for patients/clients across the continuum of care.

      Job Description

      Collaborates with healthcare team and other professionals to coordinate and ensure timely sequencing of care. Integration enhances patient flow and interdisciplinary communication promotes early intervention. Supports patients/clients and other family members by removing barriers of care.

      Seeks to evaluate outcomes based on an integration among established clinical, financial and utilization data. Functions in structured and unstructured health care settings described as a geographic and/or situational environment that may not have established policies and procedures. Utilizes independent nursing judgment when integrating health care. Knowledge of and utilizes appropriate age-related patient care protocols relating to the physical and psychological needs of pediatric, adolescent, adult and geriatric patients according to department scope of service.

      May serve as a key link between providers, clinics, insurance concerns, communication, inpatient floors and outside facilities. Establishes and maintains good interpersonal relationships with those with whom has contact. Able to work with growth and development needs of client populations in clinical area. Strong communication skills are required. Coordinates, facilitates and negotiates with others. Monitors, evaluates and trends patient responses utilizing structured care methodologies. Work closely with ancillary and support services. Ability to solve problems, identify and implement process improvements when needed. Self-motivated, working with little direction. Ability to facilitate groups, multidisciplinary teams, and implement best practices when necessary.

      Functions within the scopes and standards of nursing practice as outlined in the Nurse Practice Act and Administrative Rules in state of practice and licensure. The Sanford Professional Nursing Practice recognizes the Scope and Standards of Practice and the Code of Ethics for Nurses with Interpretive Statements as published by the American Nurses Association as the foundation of nursing care delivery and professional conduct.

      Qualifications

      Graduate from a nationally accredited nursing program preferred, including, but not limited to, Commission on Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN), and National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA). Bachelor’s degree in nursing preferred.

      Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state. Obtains and subsequently maintains required department specific competencies and certifications.
