What to Expect (Job Responsibilities):
- Provide direct patient care in collaboration with a physician, managing a patient population
- Conduct evaluations and management of acute/chronic illnesses, including wellness visits and hospital follow-ups
- Order, interpret, and communicate diagnostic tests and results
- Facilitate appropriate referrals to specialists or community-based agencies
- Educate patients and staff to maintain or restore physical and emotional health

What is Required (Qualifications):
- Master's Degree in Nursing or related field for Nurse Practitioners, or completion of an AMA approved Physician Assistant program
- Active New York State NP or PA certification and licensure in good standing
- Minimum of 5 years of experience as an RN, with 1-3 years of leadership experience preferred
- Demonstrated clinical competence and superior communication skills
- Must have applied for DEA registration prior to hire/start date

How to Stand Out (Preferred Qualifications):
- More than 2 years of active primary care practice experience
- Strong passion for primary care, even if experience is less than 2 years
- Experience in continuous quality improvement initiatives
