      Position Summary

      Collaborates with all members of the health care team, patient and family (or significant others) to coordinate timely and efficient assessment of psychosocial and discharge planning needs and formulation of patient and family centered discharge plans. Serves a resource to others in work-team and department.

      1. Provides psychosocial assessment / intervention to assist patients/families (including clinically complex)
        • Provides age specific care to patients/families in all age groups.
      2. Develops, documents and implements a comprehensive plan based on identified psychosocial needs and/or discharge needs.
        • Develops plans with attention to individual patient and family goals
        • Provides linkage to community resources and UH network services to facilitate optimal care
      3. Communicates/collaborates with the work team using a multidisciplinary approach.
      4. Provides guidance/consultation to peers/team regarding routine psychosocial issues and /or discharge planning issues.

      • Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients. Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace.

      Position Requirements:

      · Two (2) years’ experience, in a hospital setting desirable

      · Computer skills

      · Theoretical and clinical knowledge regarding health care delivery

      · Multi-tasks and prioritizes work

      · Communicates effectively with persons of various backgrounds

      · Demonstrates strong interpersonal skills and excellent communication skills, both written and verbal

      · Exhibits strong organizational skills and work ethic

      · Demonstrates a professional demeanor when responding to patients/customers

      Education

      Bachelor’s degree from an accredited school of Social Work

      Required License

      Licensed by the State of Ohio Counselor, Social Worker, Marriage and Family Therapist Board at the (LSW level)

      Position Details

      At least 2 years of experience is strongly preferred. Previous work in medical social work is also preferred. This PRN day shift position is located at the Mentor Seidman Cancer Center. Coverage is for PTO, LOA and staff shortages. Prefers someone that can also travel to Beachwood and Chardon.
