- Develop a positive rapport with each resident through routine contact and communication
- Provides support and assistance by completing care coordination tasks such as; completing referrals for services and residency forms
- Assist in the implementation of case plans and document the client interactions in HMIS and/or OCTO database
- Immediately report serious incidents, allegations, and sensitive program information to the Program Manager
- Complete incident reports
- Work with case management team to ensure all required service delivery is documented and to identify strengths and weaknesses in data entry and quality
- Respond to immediate client needs, help de-escalate crisis situations, help resolve conflicts and enforce program rules
- Conduct room inspections and locker inspections per program rules to ensure cleanliness
- Conduct regular and random drug screenings per program rules and program contracts
- Complete medication logs for clients and sign off on client medication logs, assist clients in keeping track of medication refills
- Participate in program team meetings, staff meetings, supervision meetings as required and requested per contracts
- Build and maintain quality working relationships with colleagues, external and internal stakeholders
- Participate in program quality improvement activities that improve program policies, procedures and client and program outcomes
- Adhere to agency policies and procedures
- Other duties as assigned
Position Summary
The Program Associate in an interdisciplinary team to provide services for women in N Street Village’s programs that include day services, low barrier shelter, temporary shelter, and permanent supportive housing.
We are seeking a reliable candidate who will work weekend overnights in our Miriam's House Permanent Supportive Housing Program (PSH). The incumbent will work Fridays (11 pm - 7 am), Saturdays (11 pm - 7 am), and Sundays (11 pm -7 am). Additional hours may be granted based on program coverage needs.
