      Main duties of the job

      ABL Health have been commissioned by Sandwell metropolitan borough council to deliver a high quality and Tier 2 Adult Weight management Service across the Sandwell borough. The Project Lead will assist the Head of Service for the development and operational management of the Adult Weight Management Service in the community, primary care, and secondary care settings. This is a leadership role where the post holder will have a real influence on the clinical and operational excellence of these elements of the service. The post holder will take a pro-active lead in building relationships, partnerships and alliances in the community, secondary care, and primary care settings, and supporting health lifestyle practitioners in these areas to deliver outcomes that benefit clients referred to the service. They will be responsible for the line management of staff covering the Sandwell borough.

      The post holder must have extensive knowledge and hands-on experience in public health or behaviour-change settings.

      Main Duties and Responsibilities

      The post holder has responsibility of delivering the following key result areas:

      Provide a leadership role in service planning, development, implementation, and coordination of the stop smoking service.

      To develop and coordinate a team of Healthy Lifestyle Practitioners to ensure they provide evidence based effective interventions to Sandwell residents.

      Line manage a team of Healthy Lifestyle Practitioners and be responsible for supervision and day to day management.

      Actively involved in the promotion and advertisement of the Your Health Sandwell.

      To support the Healthy Lifestyle team with their professional development ensuring audits and observations are scheduled regularly.

      To take accountability and responsibility for KPIs and ensure weekly/monthly updates are provided to the Head of Service.

      Work collaboratively with key stakeholders in the community, primary care, and secondary care to ensure a seamless referral pathway and feedback system is adhered to and improved where necessary.

      Input to multi -disciplinary meetings as required.

      Work in partnership with community organisations, primary care, secondary care, and community stakeholders to disseminate campaigns related to the adult weight management.

      Increase referrals by promoting and communicating with other professionals.

      Contribute to the social values of the service.

      Work with indirect minimal supervision.

      Work within agreed boundaries and relevant policies and standard operating procedures.

      Plan and organise workloads effectively

      Where necessary, provide session cover and the delivery of evidence-based healthy lifestyle support, in line with NICE guidelines

      Ensure robust client records are up to date at all times.

      Use reflection and evaluation to improve service delivery.

      Deliver MECC/VBA training to appropriate professionals and other stakeholders.

      This role will develop over time and therefore it is expected that post holder will have additional responsibilities
