What to Expect (Job Responsibilities):
- Initiate and manage clinical referrals for participants in disease management or health programs
- Act as a liaison between hospitals, health plans, physicians, patients, vendors, and other referral sources
- Review referrals for completeness and follow up for additional information, assigning referrals to staff as necessary
- Verify insurance coverage and obtain authorizations from insurance plans
- Maintain a database of insurance companies for coverage verification and patient eligibility requirements

What is Required (Qualifications):
- High School Diploma or equivalent
- Minimum of 1 year of experience in a healthcare environment or a combination of education and experience providing equivalent background
- Ability to travel to worksite and other locations as necessary

How to Stand Out (Preferred Qualifications):
- Experience in case management or disease management services
- Familiarity with insurance verification processes and healthcare systems
- Strong communication and interpersonal skills
