What to Expect (Job Responsibilities):
- Manage a small caseload of Medicare patients with chronic conditions and assist fellow nurses as needed
- Utilize specialized care management software to conduct monthly patient calls and promote positive health outcomes
- Build rapport with patients and coach them on self-management strategies and SMART goals
- Implement and update the Plan of Care, including medications and preventive care measures
- Conduct Transitional Care Management activities for high-risk patients to reduce readmissions
What is Required (Qualifications):
- Current COMPACT nursing license
- Minimum of 3 years of experience as a Registered Nurse
- Fluent in English; Spanish-speaking is a plus
- Strong organizational, communication, and telephonic skills
- Ability to work independently with minimal supervision and meet performance metrics
How to Stand Out (Preferred Qualifications):
- Experience in Case Management or Chronic Disease Management
- Certified Diabetes Educator
- Familiarity with Motivational Interviewing or behavior change communication techniques
