What to Expect (Job Responsibilities):
- Collaborate with clinic team members to provide evidence-based, patient-centered care through virtual solutions
- Coordinate with the care team to keep patients informed about their care plans and health improvement goals
- Liaise between patients, families, and healthcare agencies to enhance communication and facilitate whole-person care
- Monitor patient progress and adherence to treatment goals, updating care plans as needed
- Assist in enrolling patients in Remote Patient Monitoring and provide education regarding their care plans

What is Required (Qualifications):
- Associate’s degree in nursing
- Current Ohio Registered Nurse license
- CPR/BLS certification
- Experience in performing clinical and medical procedures
- Knowledge of triage procedures and appropriate standards for patient care

How to Stand Out (Preferred Qualifications):
- Bachelor of Science in Nursing
- ACLS certification
- Two or more years of nursing experience in an outpatient setting
- Prior experience in telehealth or virtual care settings
- Experience in clinical case management
