- Undertake clinical tasks agreed within competence to the specific clinical area
- Support individuals with all aspects of care including daily living, providing person-centred care and promoting health and independence.
- Take responsibility for a caseload of patients and contribute to ongoing assessment recognising when it is necessary to refer to other professionals as appropriate
- Re- assesses, evaluate and report back any concerns, variation the registered nurseson the condition, behaviour, activity and responses of individuals.
- Undertake effective monitoring, review and escalation of a person's presentation and symptoms relating to physical, mental, cognitive, behavioural and emotional components.
- To implement a plan of care which is in accordance with the patient's individual care needs and the predicted date of patient discharge
- To have an active role as a member of the multidisciplinary team demonstrating understanding of the roles of a range of professionals and carers and ensuring timely and effective collaboration/ communication of professionals and carers from other organisations and settings.
- Ensure clear, concise, timely, accurate and legible records and all communication is maintained in relation to care delivered adhering to local and national guidance.

Job summary
The Nursing Associate will work independently to implement care packages under the supervision of a Registered Nurse. To support the delivery of patient centred approaches and provide compassionate, safe and effective care to a designated caseload /cohort of patients.
The Nursing Associate will provide holistic and person-centred care and support for people across the sphere of nursing and always communicating effectively within the team, with other health care professionals and with the patient family, carers/ visitors.
The Nursing Associate will work independently but with the support and leadership of registered nurses, working within aspects of the nursing process to ensure high standards and a safe environment are maintained.
Hours per week: 37.5
