Rehabilitation Support Worker - Band 3
Trowbridge Community Team
37.5 Hours per week - Permanent
We are looking for a Rehabilitation Support worker to work within the Trowbridge Community Team. This role is for anyone who is looking for a new challenge.
You will be required to provide rehabilitation directly to patients in their own homes within primary care and work alongside Occupational Therapists and Physiotherapists. This is part of the Home First initiative designed to facilitate timely discharge from hospital settings.
You must be able to demonstrate a passion for promoting excellence in the delivery of patient care in / across different environments.
Main duties of the job
This role will bring you into the wider multi-disciplinary team alongside our partners in G.P. surgeries, acute settings and specialist services in pursuit of excellent patient care. We are a friendly team that includes Care Coordinators, Community Nurses, Occupational Therapists, Physiotherapists Healthcare and Rehabilitation support workers and Admin, delivering care to people in their own homes, including residential settings. Successful applicants will have a confidence in working autonomously as directed by trained colleagues. You will be supported by the Clinical Lead and Community Team Manager as well as having access to specialist support from Wiltshire Health and Care.
You will need to have a current UK drivers license and vehicle available to visit your caseload within our community.
