What to Expect (Job Responsibilities):
- Educate patients on disease states and engage them in their care
- Coordinate care plan objectives to improve patient care and health outcomes
- Work with clinical practice teams throughout all phases of the care continuum
- Collaborate with internal and external healthcare entities to coordinate health services
- Expedite the continuum of care to achieve high-quality, patient-centered services

What is Required (Qualifications):
- High School Diploma or GED required
- Community Health Worker certification preferred
- Minimum of 2 years of experience in a Community Health setting preferred
- Knowledge of Medicare/Medicaid and other insurance plans preferred
- Experience working with the Medicaid/Medicare population preferred

How to Stand Out (Preferred Qualifications):
- Bilingual in Chinese/Mandarin highly preferred
