Job Title: Remote LPN Care Manager

Job Description

We are looking for an LPN Care Manager to become a part of our team for a healthcare corporation.

Responsibilities

• Provide day-to-day Care management activities,tance to the care management team and communicating with the supervisors daily.
• Complete member assessments and develop care plans.
• Assist members with their needs and coordinate with the care management team.
• Communicate regularly with supervisors to ensure effective care management.

Requirements

• Licensed Practical Nurse (LPN) or Registered Nurse (RN) certification in the state of Florida.
• Proficiency in Care Management.
• Experience in Pediatrics and Acute Care.
• Strong computer skills, including proficiency in Microsoft Suite.

Work Environment

The position operates on a fully remote basis with a standard schedule of Monday through Friday, 8:00 AM to 5:00 PM, totaling 40 hours per week. Candidates must have a reliable Wi-Fi connection, a dedicated workspace, and a compatible smart device to fulfill the role's requirements.

Pay and Benefits

The pay range for this position is $30.00 - $36.06/hr.
