Job Description

- Creates dietary recommendations to care teams in the form of consults for patients who meet eligibility criteria
- Counsel patients, and patients’ families on prescribed diets and monitoring adherence and response to diet therapy
- Evaluates a patient’s nutritional status through medical records, interviews, and diet histories
- Develops meal plans that consider a patient’s preferences and budget
- Tracks patient’s progress and makes changes to meal/nutrition plans as needed
- Develops and provides nutritional educational materials to patients and families
- Collaborates with patient’s Care Team and other multi-disciplinary clinicians including: endocrinology, cardiology, pulmonology, nephrology, palliative, etc. to deliver appropriate care
- Stays current on latest research and guidelines related to food and nutrition
- Assist with clinical and operational procedures to improve patient health and minimize hospitalizations and missed treatments.
- Perform regularly scheduled evaluations of standard menus for renal patients, revising and/or updating menus as necessary
- Provide ongoing monitoring of data to determine need for timely intervention and follow-up
- Assist with program’s target goals for patient outcomes in accordance with quality patient care and company goals
- Must perform initial assessments in accordance with state regulations
- Nutrition reassessments shall be conducted in accordance with state guidelines or more often if indicated
- Assure follow-up progress notes are written in each patient’s record
- Assure documentation corresponding with the patient’s care plan is noted
- Educate staff and provide in-service training in relation to patient dietary care
- Assist with all programs target goals for patient outcomes in accordance with quality patient care and company goals
- Participate in staff meetings as required. Attend in-services and continuing education sessions in compliance with company policy and procedure
- Assist with staff training as requested
- Bachelor of Science Degree in Dietetics, Nutrition or Food (Required)
- 3 to 5 years' experience in clinical nutrition as a Registered Dietician (Required)
- Registered Dietitian License (RD/RDN) certification showing proof registered with Commission of Dietetic Registration (Preferred)
- 1 year of dialysis experience (Preferred)
- Willingness to become licensed in multiple states as company and patients needs require
- Ability to effectively communicate in writing as well as verbally
- Must have basic computer skills; proficiency in all NRC clinical applications required within 90 days of hire
- Must be able to organize time and tasks well
- Ability to meet minimum hospital/agency hiring standards which may include additional background clearances and orientation requirements if applicable to the program assigned to for employment
- Must meet any practice requirements for the relevant state

Position: Renal Dietician
The Dietitian’s role is to assure quality nutritional care of patients and their families with chronic renal disease. The Renal Dietitian assesses, monitors, and educates CKD and ESRD patients on their ongoing nutritional needs while they receive dialysis and other required treatments. The Renal Dietitian's dietary consultation is a key aspect of improving care for our CKD and ESRD patients. The Dietitian will support our patients in following an accessible and kidney-friendly diet.
