What to Expect (Job Responsibilities):
- Support complex patients in navigating the healthcare system
- Identify and address the medical, social, emotional, and financial needs of patients
- Coordinate and facilitate quality, consistent, cost-effective care
- Build empathetic relationships with patients and families
- Assess patients and identify unreported medical conditions or changes

What is Required (Qualifications):
- Passion for delivering care to patients with complex, chronic illnesses
- Current RN License, BSN preferred
- Minimum of 5 years of RN experience
- Current CPR or BLS certification
- Current driver's license and willingness to travel within assigned territory
- Intermediate computer skills, including proficiency in MS Word, Excel & Outlook

How to Stand Out (Preferred Qualifications):
- 2+ years of Case Management or Chronic Care Management experience
- 1+ years of utilization management experience
- Spanish-speaking preferred in some markets
