Job Description

- Participate in daily interdisciplinary team meetings
- Complete periodic health assessments
- Assess acute needs and illness in the clinic, participant’s home, and facilities (ALF/SNF)
- Advanced Care Planning
- Patient and caregiver education
- RN-Nursing Diploma or Degree from accredited School of Nursing or University
- Current license to practice as an Registered Nures (RN) in the State of Michigan
- Minimum of one year of experience working with the frail elderly, or long-term care population
- Previous home care experience or long-term care experience is preferred

Overview
The LifeCircles clinical team delivers primary care, palliative care, and end-of-life care for patients aged 55 and older. This position is perfect for the RN who wants to be patient-centered, creative, and collaborative in their approach to care. This position works closely with an interdisciplinary team and other RNs in a well-supported, team-based environment.
