What to Expect (Job Responsibilities):
- Manage a caseload of patients with diabetes through telephonic and virtual interactions

What is Required (Qualifications):

- Bachelor's Degree in Nursing
- 5 years of patient care experience with a focus on chronic illness as a certified diabetes educator
- Knowledge of best practice standards for diabetes care
- Excellent communication, critical thinking, and organizational skills

How to Stand Out (Preferred Qualifications):
- 0-2 years of experience as a certified diabetes educator in a healthcare-related setting
