      Overview:

      Join Our Compassionate Team as a Start of Care Nurse
      At Mercy’s Home Health, we believe in providing outstanding care that makes a significant difference in the lives of our patients. We are currently seeking a dedicated Start of Care Nurse. As the first point of contact for our new patients, you will have the opportunity to set the tone for the quality and compassion that our agency is known for.

      Key Responsibilities:

      - Evaluate the patient’s home environment to determine the suitability for home care and identify any modifications needed to ensure safety.

      - Conduct Comprehensive Assessments: Utilize your expert clinical skills to perform detailed evaluations of patient health, crafting tailored care plans that address physical, mental, and environmental needs.

      - Develop and Coordinate Care Plans: Work collaboratively with a multidisciplinary team to ensure seamless care delivery, initiating necessary referrals and adjustments to maximize patient health outcomes.

      - Educate and Empower: Teach patients and their families crucial self-care techniques, turning challenges into opportunities for growth and learning.

      Qualifications:

      • Required Education: Nursing Degree from an accredited school
      • License:
        • Oklahoma RN licensure
        • Valid driver's license
      • Certifications:
        • CPR certification
        • Basic Life Support certification through the American Heart Association or successful completion of course within 30 days of hire- LPN-IV Certification must be previously completed or successful completion of course within 3 months of hire (range from upon hire to within 1 year)
      • Experience: At least one year experience in Home Health.
