What to Expect (Job Responsibilities):
- Collaborate with customers to create and document care plans in the medical management system
- Establish goals and interventions tailored to customer needs in partnership with providers
- Utilize motivational interviewing and shared decision-making to help customers achieve optimal health
- Coordinate with internal partners and providers through various communication methods
- Track daily activities and adhere to standard operating procedures to meet organizational goals

What is Required (Qualifications):
- Active unrestricted Registered Nurse (RN) license in a state or territory of the United States
- Minimum of two years of full-time direct patient care experience as an RN
- Flexibility to work days or evenings based on business needs
- Completion of training within the communicated time limit as required for the role
- Demonstrated evidence of continuing education to maintain clinical expertise and certification as appropriate

How to Stand Out (Preferred Qualifications):
- Availability to work non-standard shifts with a potential 12% shift differential
- Active and unencumbered RN License in the State of Residence
- Ability to obtain a URAC-recognized certification in case management within four years of hire
- Strong communication skills, both telephonic and digital
- Proficiency in Microsoft Word, Excel, and Outlook, with the ability to conduct thorough internet research
