Job Description

- Perform telephone triage and patient assessment from Agrace office or home.
- Respond to patient and family calls in a timely manner.
- Effectively triage patient/family needs via the phone; assesses need of a home visit.
- Interact in a positive, effective manner with patient, families, collaborating agencies and health care providers including phone and written communication with a focus on excellent customer service.
- Utilize data collected from the patient, family and/or significant other or other members of the interdisciplinary team and other health care professionals.
- Utilize knowledge of uncomplicated grief and the dying process as appropriate in patient and family assessment, care plan updating and intervention.
- Articulate, identify, and reports signs and symptoms of client abuse and/or neglect, suicide ideation or violence.
- Assess the need for individualized patient and family education.
- Develops and documents effective acute care planning.
- Assess pain on an on-going basis.
- Collaborate with patient, physician, and appropriate members of the interdisciplinary team.
- Use pain management skills to aggressively intervene during a pain crisis.

- Participates in mandatory educational requirements.
- Precept new staff.
- ADN, required
- Current, unrestricted Registered Nursing License in state of employment
- One-year nursing experience, preferred
- Must be at least 18 years of age
- Ability to develop, maintain, and apply knowledge of hospice and end of life process
- Ability to maintain professional boundaries with patients, families and co-workers
- Ability to create and follow plans of care, and patient, family, and clinical staff directions
- Basic Microsoft Office skills
- Intermediate electronic health records skills; Epic preferred
- Ability to work effectively and cooperatively with others
- Ability to communicate effectively verbally and in writing
- Ability to be flexible in time and task to meet organizational needs
- Ability to maintain confidentiality about patients, families, caregivers, staff, donors, and organizational intellectual property
- Ability to use proper body mechanics, safe patient handling, and standard precautions

Shift PM - 4pm- 12:30am Hours/Weekends 0.7 FTE 28 hours/week and every other weekend Holidays every other holiday rotation Hybrid position works primarily remote- orientation and one day/month in office

The RN Triage role works in a hybrid schedule with the rest of the Triage team, you will collaborate with Agrace staff, families, and patients through your Agrace provided devices. You will have others to support you throughout your shift and there is a working supervisor for all shifts!

HOW YOU’LL MAKE A DIFFERENCE IN THIS ROLE (ESSENTIAL FUNCTIONS)
The Triage RN is responsible for coordinating direct and indirect patient care that is based on nursing process. The Triage RN will assess each call and determine if a visit is needed or if assistance can be handled over the telephone. The Triage RN maintains accountability for nursing practice, patient outcomes, nursing assessments, interventions, evaluations documentation and coordination of care. This position ensures hospice services are patient and family centered and consistent with philosophy, goals, standards, regulations and policies of the program and Agrace.
