      Job summary

      Mildmay is looking for an enthusiastic and innovative Social Worker with experience of working with adults. As a Hospital Social Worker, you will play a key role in supporting adults with complex needs. You will assess, identify and manage risk, lead on complex cases, and provide expert guidance to the internal team and external partners. The post holder will also provide leadership and supervision to the Social work assistant.

      Main duties of the job

      To deliver a high standard of service to all patients at MMH referred to the team, working autonomously.

      To provide a highly specialist assessment and evaluate the needs of inpatients at MMH and ensure that these needs, and those of families and carers, are met in a manner which promotes safety, independence and dignity.

      To be a source of support and expert information and advice to all patients, advising them about DWP, NHS and local authority benefits, financial matters, charitable help, housing and legal issues.

      To facilitate the safe and timely discharge of patients from hospital, ensuring that appropriate agencies in the community have been advised and activated, using the procedures specified in the 2003 Community Care (Delayed Discharge) Act.

      To work with the multi-disciplinary team to monitor and minimise patient length of stay.

      To liaise with other social service departments and agencies to ensure that patients are supported through their treatment and beyond, whether home or hospital based, ensuring that a proactive

      To be constantly aware of information governance requirements, only sharing patient information with permission and if essential.

      To be a core member of the MDT attending weekly meeting, and contributing to their goal setting.

      To have a delegated caseload of patients, contributing to their MDT goal setting and planning.

      To undertake key worker duties.

      Utilise rehabilitation and teaching skills to educate the diverse and multicultural population in a range of disease specific therapeutic techniques, principles and guidelines.

      Use of outcome measures as required and further develop these in partnership with others to ensure a high quality service is delivered.

      Ensure that quality standards relating to social work are continually monitored and improved.

      To actively identify areas of service innovation and development for the Social Work service and work with the professional lead in developing this.

      • Minimum of 4 years post registration experience which demonstrates the competencies to meet the role requirements outlined in the JD.
      • Evidence of management of highly complex cases including complex manual handling issues.
      • Evidence of recent Continuing Professional and Personal Development
      • including specialist short courses.
      • Experience of applying and interpreting standardised assessment to patient group.
      • Experience of undertaking risk assessment and risk management
      • Experience of developing and delivering training Packages
      • Experience of working in an integrated health and social care team
