      Closer2Home is a specialist intensive home treatment service that supports young people with acute mental health problems in their homes by providing intensive input and support using a bespoke package of care tailored to each young persons needs. We operate 365 days a year from 8am-8pm to offer flexibility to young people at times when they need it the most.

Hospital admission can be lifesaving but often has a detrimental effect on young peoples as well. Closer2Home provides a service that helps young people in crisis to remain in their own home, in their own community where they can get involved in meaningful activities that are worthwhile to them.

      Main duties of the job

      Working as a team, you and your colleagues are responsible for designing and delivering time-limited programmes of intensive home treatment to young people with a range of needs, who would otherwise have been admitted to hospital. The team work collaboratively with community CAMHS, children's services, colleagues involved in 'transforming care,' bed and case managers and local inpatient units; using an interagency approach to provide the best care for the young people.The applicant will be expected to provide clinical leadership to their local Closer2Home team. Each team has been modelled to have an optimal number and skill mix of staff to effectively address the needs of the young people the service will support.To operate as one fully integrated team staff are employed by one 'host' Trust but have honorary contractual agreements with the other Trusts to enable them to work across the whole service.

      The transformation will happen in staggered stages; Hampshire CAMHS, part of Sussex Partnership NHS Foundation Trust joined Southern Health in February 2024, with Isle of Wight NHS Trust mental health and community services transitioning in early May 2024. The final combination of Southern Health and Solent Trusts is anticipated to occur in July 2024, when the new organisation will be formed.

      Our new Trust will continue to cater to the unique needs of different communities, making healthcare across the county more accessible, as well as offering staff more opportunities for career development, training and partnership working.
