      Job summary

      Whitby Coast and Moors Primary Care Network are looking to recruit an enthusiastic and compassionate Senior Nurse. Our PCN is setting up a new multidisciplinary holistic care team to provide dedicated support and care for patients with complex needs. The team will care primarily, but not exclusively, for older adults.

      You will have the opportunity to share in the development of this new team which will focus on health, wellbeing and independence.

      You will need to have clarity of professional identity and be comfortable working with a degree of uncertainty as new services are tested and developed.

      In this new team there will be plenty of opportunity to use your service development, leadership and excellent clinical skills as you work with others to improve outcomes for patients.

      Main duties of the job

      This role is part of our new complex care team offering holistic care to patients with complex needs, often elderly patients living, for example, with frailty, long term conditions, or social and mental health needs.

      We are looking for nurse who is enthusiastic about playing an active part in our complex care team that will work with our evolving integrated neighbourhood team (INT). This is an exciting role, and we are looking for a candidate who will exemplify good team working and effective respectful communication with all team colleagues across professional boundaries such as primary care, community services, social services, mental health, and the voluntary sector.We regard empathy for our older patients and a desire to be guided by what matters most to them to be important attributes.

      Role to be responsible for the implementation of processes for the effective management of patients with long-term conditions using evidence-based practice including care for elderly and housebound patients. Working as a senior member of the practice multidisciplinary team, the post holder will ensure nursing services are delivered effectively to the entitled patient population.

Our member practices are Whitby Group Practice, Esk Valley Medical Practice, Staithes Surgery and Sleights and Sandsend Medical Practice. Our population faces particular challenges relating to higher than average chronic disease, coastal deprivation and rural isolation.

      Core Clinical Function

      The following are the core responsibilities of the Senior Nurse. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels:

      1. To be instrumental in the support of the Infection Prevention Control (IPC) lead and to work to the IPC requirements by providing audits, training, support and evidence of compliance to the practice manager

      1. To support in the publication of the IPC annual statement in conjunction with the practice manager

      1. To develop, implement and embed an effective care management programme for the frail and housebound patients registered at the practice

      1. To assess the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence-based practice

      1. To develop, implement and embed health promotion and wellbeing programmes

      1. To implement and evaluate individual treatment plans for chronic disease patients that promote health and wellbeing

      1. To identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to patients health

      1. As required, to provide routine nursing care to patients in accordance with clinical based evidence, NICE and the NSF

      1. As required, to diagnose and manage acute and chronic conditions

      1. Provide wound care (ulcer/Doppler, etc.) to patients

      1. To review patient medications to enhance compliance

      1. To provide guidance to patients in the use of prescribed and over-the-counter medications regarding side effects and dosages

      1. To co-ordinate the provision of travel medicine services

      • Currently working as a specialist nurse practice

      • Experience of working autonomously within the specialist area

      Desirable

      • Extensive and up to date post registration experience at band 7 or above
      • Diabetes Nursing Qualification e.g. Certificate in Diabetes Care or equivalent
      • Teaching / Mentor Qualification
