What to Expect (Job Responsibilities):
- Serve as the link between patients and healthcare personnel to ensure efficient care transitions
- Conduct assessments on patients in skilled nursing facilities using clinical skills and CMS criteria
- Collaborate with healthcare teams to establish optimal discharge plans
- Monitor patients' progress toward discharge goals and assist in resolving barriers
- Maintain accurate documentation and adhere to organizational policies and procedures

What is Required (Qualifications):
- Active, unrestricted registered clinical license as a Registered Nurse, Physical Therapist, Occupational Therapist, or Speech Therapist
- 3+ years of clinical experience in a relevant field
- Proficiency in Microsoft Outlook, Excel, and Word
- Access to reliable transportation and high-speed internet from home
- Dedicated, distraction-free home office space

How to Stand Out (Preferred Qualifications):
- Understanding/knowledge of CMS, Medicare, and Medicaid guidelines/regulations
- Case Management experience
- Experience working with geriatric population
- Patient education background, rehabilitation, and/or home health nursing experience
- Strong problem-solving, conflict resolution, and communication skills
