      Social Worker – Purley Locality Team

      Grade 10 – (£40,833 - £42, 840)

      If you are a qualified social worker and registered with Social Work England why not come and join our exciting team working with older people in Croydon.

      We need a committed and knowledgeable social worker to help us with our core business – empowering and supporting people who need signposting, community support, or even some direct help from the Council. We have a strong operating model of a strengths-based approach.

      The Council Leadership has made clear their support for our statutory functions, including Adult Social Care and Health. We know where our priorities are as a council.

      We have an established and stable team of social workers, health and wellbeing assessors and a team manager to work with you and support you. Our vacancy rates are low, and we rarely use agency staff, preferring to concentrate our efforts on looking after our permanent staff. This includes having a dedicated training officer supporting adult social care and health, and a Principal Social Worker to support staffs’ welfare and practice. We have also recently upgraded our electronic recording system.

      As a qualified Social Worker, you are expected to have a minimum of 1 year post qualifying experience and be prepared to take on responsibilities for the following:

      Assessment and Care Planning: Conduct comprehensive assessments of adults' needs, including physical, psychological, and social factors. Develop personalized care plans in collaboration with service users, their families, and other professionals, ensuring that interventions are person-centered and promote independence.

      Case Management: Manage a diverse caseload of adults with varying needs, including older adults, individuals with disabilities, and those with mental health concerns. Provide ongoing support, advocacy, and coordination of services to ensure that individuals receive the appropriate care and support to meet their goals.

      Safeguarding: Identify and respond to safeguarding concerns promptly, following local policies and procedures. Work collaboratively with safeguarding partners, such as health professionals and law enforcement agencies, to ensure the safety and well-being of vulnerable adults.

      Multi-Disciplinary Working: Collaborate effectively with colleagues from various disciplines, including health professionals, housing officers, and voluntary organizations, to provide holistic support to service users. Participate in multi-agency meetings and case conferences to coordinate care and facilitate joint working practices.

      Promotion of Independence: Support adults to maintain or regain their independence by accessing community resources, social activities, and housing options. Provide practical assistance with tasks such as budgeting, housing applications, and accessing benefits to promote self-sufficiency and empowerment.

      Record Keeping and Reporting: Maintain accurate and up-to-date records of assessments, care plans, and interventions using electronic systems. Prepare clear and concise reports for case conferences, court hearings, and other formal reviews, ensuring compliance with legal and regulatory requirements. You will also form part of the duty rota within the team.

      Duty Rota working: Participate in Locality duty rota.

      There is 1 vacant post in Purley Locality Team, this will be based in Bernard Weatherill House. You will work with other professionals in an integrated service providing strengths-based assessments to promote independence and support people to remain living in their own homes.

      Must be a car driver with full British Drivers Licence

      Social Work England registered.
