As a vital member of our interdisciplinary Home Health and Hospice team, the Social Worker plays a crucial role in facilitating optimal patient outcomes through a collaborative approach. This position involves assessment, planning, facilitation, and advocacy to address individual health needs effectively. The Social Worker will ensure the efficient and appropriate use of resources across the continuum of care while promoting evidence-based practices and compliance with professional standards.

This position will primarily serve patients who reside in and surrounding the cities of St. Cloud, Clear Lake, Becker, Monticello, Big Lake and Buffalo.

Schedule:

• Full-time | 72 hours per two weeks
• Days | Monday-Friday | 8:00 am – 5:00 pm
• No weekends or holidays!

Pay and Benefits:

• Staring pay beings at $27.95 per hour; exact wage determined by years of related experience
  • Pay range $26.59 per hour - $39.88 per hour
• Milage reimbursement and extra pay for working weekend shifts!

Qualifications:

• Current Licensed Social Worker (LSW) in Minnesota
• Bachelor's degree in Social Work -OR-
• Bachelor's degree in Sociology with 5 years of social work experience
• Valid Minnesota driver’s license, a driving record in good standing, and insurance liability coverage and reliable transportation

To be eligible for the hiring bonus, you must be an external candidate who is hired into an eligible full time or part time benefited position. Previous employees qualify if they have not worked for CentraCare for at least 9 months at the date of hire.

To be eligible for the relocation assistance, you must accept a position that offers benefits and may not have worked for CentraCare in the last 9 months. You must currently reside more than 50 miles away from the Marketplace office in Waite Park, MN and relocate to within 30 miles of it within 120 days of employment.
