      Responsibilities

      • Provide direct discharge planning services and assistance with psychosocial services for the patient and family.
      • Collaborate with physicians and other members of the healthcare team to develop a discharge plan in relation to the patient's health care needs.
      • Utilize broad experience and knowledge of appropriate healthcare resources for patients of all ages.
      • Promote continuity through communication of the patient's plan of care.
      • Report to the Clinical Resource Management Coordinator.

      Qualifications

      Job Requirements

      • Master of Social Work (MSW)
      • Clinical emphasis from a CSWE accredited graduate school
      • Kansas licensure at the LMSW level

      Preferred Job Requirements

      • Two semesters of graduate field experience in healthcare setting for a new graduate or previous experience in a healthcare setting for an experienced graduate
