- Provide direct discharge planning services and assistance with psychosocial services for the patient and family.
- Collaborate with physicians and other members of the healthcare team to develop a discharge plan in relation to the patient's health care needs.
- Utilize broad experience and knowledge of appropriate healthcare resources for patients of all ages.
- Promote continuity through communication of the patient's plan of care.
- Report to the Clinical Resource Management Coordinator.
- Master of Social Work (MSW)
- Clinical emphasis from a CSWE accredited graduate school
- Kansas licensure at the LMSW level
- Two semesters of graduate field experience in healthcare setting for a new graduate or previous experience in a healthcare setting for an experienced graduate
