      Job summary

      CCOT/MET Band 7 Secondment

      An opportunity has arisen for an experienced Critical Care Nurse to join the Critical Care Outreach Team/Medical Emergency Team for a 6-month secondment. This will be a part time role of 24 hours, so full time candidates will work their remaining hours on Critical Care.

      The successful candidate will have the chance to gain experience in the care and rehabilitation of patients stepped down from Critical Care to the general ward areas and develop their teaching and educational skills by participating in ward based clinical teaching and support of ward staff. There will also be the opportunity to get experience as a faculty member on the BEACH and AIM courses.

      Early in the secondment, we will aim to get the successful candidate on the ALS provider course (if they don't already hold this qualification) and they will work as a Nurse responder on the Trust wide MET to calls to all acutely deteriorating patients and cardiac arrest situations.

      They will gain experience as part of the AKI and Sepsis surveillance service and we will provide the opportunity during the secondment to develop the clinical skills required for the CCOT/MET Specialist Nurse role and those within the National Outreach Forum Critical Care Outreach Practitioner Framework.

      As this is a secondment all applicants will require the support of the Critical Care Matron to apply and be able to be flexible with off duty across both teams.

      Main duties of the job

      This is a Specialist Nurse role on the front line of acute care, working across the Aintree site. The role is multi-faceted, including review and care of Critical Care step down patients, working as the primary Nurse responder on MET calls and also providing Sepsis and AKI surveillance support for all at risk patients identified on our database. The successful candidate will be a dynamic enthusiastic Registered Nurse able to work autonomously and as part of the MDT. They will have good decision making and critical thinking skills. They will also have a keen interest in education and be able to support and train junior ward Nursing and Medical staff and maintain good professional relationships across the organisation

      Job responsibilities

      Provides expert clinical knowledge to Nursing and Junior Medical Staff and acts as a clinical role model, demonstrating skills in identifying patients at risk and initiating treatment to critically ill patients regardless of location

      Perform autonomous comprehensive assessments of critically ill patients, plan, implement and evaluate care delivery according to changing health needs and varying levels of complexity

      Independently identifying patients at risk, both prior to and following discharge from Critical Care

      Maintenance of track and trigger score and escalation when required

      Active member of ward based MDT working to rehabilitate Critical Care step down patients, identify psychological problems, offering informal support and where appropriate referring on for formal investigation or follow up by the relevant specialty

      Coordinate and maintain effective communications between Critical Care and other ward/departments, managing conflicting views where necessary

      Works collaboratively with other specialties to provide specialist care for patients recently stepped down from Critical Care, those at risk of acute deterioration and those requiring specialist support for example, tracheostomy and laryngectomy patients in general/cohort wards

      Responds as part of the Aintree Medical Emergency Team, coordinating calls and assigning Team members roles and responsibilities.

      Initiates prescribed treatment to the acutely ill ward patient during an emergency and ensures full management plan delivered and evaluated

      Coordinates the inter hospital transfer of acutely ill level 1,2 and 3 patients for investigations or to an area for a higher level of monitoring

      Reviews and responds to patients alerting with Acute Kidney Injury (AKI) and Sepsis as part of the Trust Avoidable Mortality Reduction Programme instigating use of appropriate care bundle, treatment and monitoring and meeting all of the role specific competencies for the AKI Specialist Nurses
