      Job summary

      Following investment into the community hospitals wards we have an exciting opportunity to join our team. This post is a 12 month fixed term role. We are seeking a highly motivated and enthusiastic Band 6 Occupational Therapist to join our therapy rehabilitation team based at in the south localities ( Richardson and Sedgefield Community hospital). The base site will be Richardson community hospital.

      Applicants must be prepared to participate in a 6 day week service, covering bank holidays and Saturdays on a rota basis.

      The successful applicant will be required to work across the south community hospitals in the Trust.

      Main duties of the job

      This post is for a specialist occupational therapist full time.

      The service will work over 6 days a week.

      The successful applicant will independently manage a caseload in the community wards providing holistic, short term, goal led intervention following a comprehensive assessment. These patients will present with a wide range of complex problems and will allow you to develop a highly specialist knowledge

      Promoting a Discharge to Assess model of care, patients will have their discharge facilitated by therapists at the most optimum point in their rehabilitation pathway.
