This position exists to provide comprehensive, value based, medication-related care for patients who are prescribed a specialty medication (example dermatology, rheumatology, gastroenterology, oncology, multiple sclerosis, etc.) to treat their chronic conditions. This position will mainly support inflammatory conditions but may have required coverage in additional disease states. This individual will collaborate with healthcare professionals to implement care plans in order to achieve goal patient outcomes. Key job duties may include but are not limited to, clinically assessing prescribed therapies, obtaining insurance authorization for therapies, conducting education and follow up assessments, and providing oversight to technician team members. Additionally, we are seeking an individual competent at providing drug information support for healthcare teams, ensuring continuity of care for patients, and identifying and resolving potential challenges in the medication access process. This pharmacist will be important for the expansion of specialty services across the state of Indiana. Successful completion of an ASHP accredited PGY1 pharmacy residency is preferred.
• Requires current Indiana License or eligibility for Indiana licensure as a pharmacist.
