- Delivering a consistent, coordinated approach to assist patients to identify potential needs at the point of discharge to avoid unnecessary delays in transfer of care
- Working collaboratively with external and internal partners for the benefit of the patients to ensure smooth transition from hospital and reduce risk of readmission
- Transparently providing information and expert advice to patients, carers and other professionals
- Reporting all delayed transfers of care, for criteria to reside reporting .
- Current first level registration in General / Adult nursing or Mental Health nursing
- Experience in an acute hospital environment

Job summary
An exciting opportunity has arisen for a Band 5 Staff Nurse to join our Transfer of Care Hub. We are looking for an enthusiastic and experienced Nurse who has excellent communication and assessment skills, is passionate about patient flow and able to work at a fast pace in both a changing and challenging environment.
The successful candidate will be responsible for completing assessments for patients to facilitate discharge from hospital through a Discharge to Assess model. We are looking for individuals who wish to achieve best patient outcomes on discharge from hospital embracing the Home First approach.
Hours per week: 25.5 hours.
Maternity leave cover, f ixed term contract until the end of June 25.
Secondments will also be be accepted, please ensure permission is obtained from your line manager before applying.
Main duties of the job
The post holder will facilitate initial proactive assessments for inpatients in order to facilitate a safe, effective discharge from hospital and prevent avoidable readmission to hospital.
