Participate in team meetings & partner/stakeholder engagements, as appropriate
Support the team by offering clerical assistance as required, for example:
Scheduling meetings, capturing action items, developing communications, etc.
Capturing and synthesizing findings from stakeholder consultations
Complete analysis to support priority integrated care projects
Developing summaries and visualizations of evaluation outcomes
Support the development of meeting materials and presentations
Communicate effectively and concisely, both orally and in writing
Use their organizational, time management and prioritization skills to navigate their assignments
Demonstrate a positive attitude and an enthusiasm to learn and flexibility to support a variety of assignments
Grade 12 Diploma
Some university/college is an asset
Experience with MS Word, Excel, PowerPoint
Ability to communicate clearly, work in a team and independently
Excellent written/oral communication skills.
Ability to learn quickly

The Integrated Care team within the Population Health & Value Based Health Systems division at Ontario Health (OH) works closely with OH regions and Ontario Health Teams (OHTs) to support the effective implementation of programs and initiatives to advance integrated, value-based care via a population health management approach in communities across the province.
The Integrated Care team works with many internal and external partners and stakeholders to define and develop strategy, develop implementation plans, support the successful execution of these plans, seek input to refine and refresh implementation tactics, and measure and evaluate progress and outcomes.
