      Job summary

      An exciting and unique opportunity has arisen to join Tees Liaison Psychiatry based within the acute hospital setting.

      Do you have a passion for delivering person centred care with a recovery focus?Are you abletoevidenceexperience of working with individuals with complex mental health needs and or behaviours that challenge?

      Main duties of the job

      The successful candidate must have completed the Diploma in Health and Social Care Level 3 or equivalent. Candidates must also have Literacy and Numeracy level 2 or equivalent and must complete the Trust Healthcare Assistant competencies within their probationary period (first 6 months in role).

      You will be a highly motivated individual with a broad range of experience and contemporary understanding of Adult/Older People with Mental Health problems. You will be able to demonstrate a passion about Adult/Older Persons Mental Health and an understanding of the needs and issues of older people with mental health problems, who are receiving care and treatment in acute in-patient wards. You will work collaboratively with the whole of the liaison team.

      Working hours are over 7 days, however candidates should be flexible to meet the needs of patients.

      Your key responsibilities will include monitoring and reviewing intervention plans identified by the assessor, and to review specific patients mood, behaviours, wellbeing, medication etc. You will be expected to support individual patients on the acute medical wards along side the acute trust staff by modelling your skilled practices and educating the acute trust staff to enable a person centred, individual approach to care delivery, this may also include supporting complex discharge of people into the community by working in a collaborative approach with community services.
