- Assist staff in accessing service information and resources to help serve consumers in need of housing more effectively.
- Collaborate with mental health professionals, social workers, and case managers to ensure appropriate level of care for consumers.
- Assist consumers in developing and implementing housing stability plans and accessing appropriate mental health services.
- Follow up on Tennessee Anytime, verifying eligibility of consumers.
- Assess consumer housing needs to match them with appropriate level of housing resources. See Payer process for entry into the selected home.
- Coordinate the housing application and placement process, including reviewing psychiatric evaluations to determine medical need for the appropriate level of care.
- Complete and submit requests for authorizations/re-authorizations to each insurance provider.
- Send Housing referral information on consumers who are reported to have a history of violence to the Enrollment Committee for review prior to acceptance into supported housing.
- Responsible for obtaining consumer finances, review and enlist with Representative Payee, and ensuring ongoing payments to Housing Provider.
- Notify Representative Payee when consumer leaves supported housing.
- Assist consumers with transition of housing when terminated from housing.
- Conduct or ensure regular follow-ups with clients are provided by housing associates to assess the consumer’s housing stability and address any emerging issues.
- Act as an Agency contact person for local service providers involved with the issues of housing.
- Assist Supported Housing Coordinator as needed in maintaining Payer spreadsheets.
- Report individual interruptions to Supported Housing Coordinator or Director of Housing.
- As needed, assist Supported Housing Coordinator with reporting data.
- Assist with maintaining on-going log of all housing referrals and placements.
- Conduct audit with representative of Wellpoint, United Healthcare, and Blue Cross Blue Shield as requested.
- Provide training for Housing Providers as needed (at minimum semi-annually).
- Assist with maintaining and updating weekly the Housing intranet site for agency on SharePoint.
- Assist with maintaining weekly report regarding housing availability for each location.
- Travel to supported houses as needed.
- Liaison for hospital Social Workers and housing providers.
- Maintain accurate and up-to-date records of client housing assessments, placements, and progress.
- Ensure compliance with privacy and confidentiality regulations when handling sensitive consumer information.
- Bachelor’s degree and/or minimum of three years equivalent experience in human services related field.
- Preferred previous experience in housing coordination, case management, or related roles, preferably in a mental health or social services setting.
- Preferred knowledge of mental health conditions, housing resources, and community support systems.

Job Title: Supported Housing Specialist
JOB SUMMARY: Assist individuals with Serious and Persistent mental illness in meeting their supported housing needs; Serve as Agency liaison for the Behavioral Health Organization relating to Level 1, Enhanced, and Medical Needy Housing. Assist with communication to other agencies and act as contact for other service organizations involved in housing and mental health issues.
