      Job summary

      We are currently seeking an additional Team lead to join our Community Nursing Team for the Darlington Locality . The team cover 8am until 9pm 7 days a week on a duty roster. The role involves leading a team to provide holistic nursing care to adult patients in their own home or care home settings and leading Sucessful applicants will be required to hold a first level nursing registration with the NMC and have good clinical skills.

      Main duties of the job

      The sucessful candidate will provide a variety of nursing care to patients; including wound care, administration of medication, palliative care, management of bowel and catheter care. The role involves assessment of patient care needs, care planning, delivery of care and review. The sucessful candidate will be required to lead the team in providing a high standard of care. This role has supervision and management of staff along with co-ordination shifts and triage duties, dealing with complaints, managing incidents and working closely with the locality Matron.

      We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.
