We have an exciting development opportunity to join the Tukes One Front Door Team as Tukes Social Support Officer .

Main duties of the job
The role of the Tukes Social Support Officer will play an integral in Navigos One Front Door service provided by TUKES by using your skills experience of mental health services to provide emotional and listening support to people with Mental Health problems.
The Social care support worker, through Tukes One front door Model, will champion Navigos vision of providing support for those struggling with social stresses, which as a result may lead to a misplaced mental health diagnosis. This includes working alongside the dedicated Tukes Social Care Navigator and Carers Assessment Officer.
Your duties will include supporting individuals with poor and enduring mental health, providing them with meaningful occupation of time within Tukes service teams on our Acute sites and Grimsby Garden Centre encompassing health and wellbeing, inspiration, and self care to assist individuals move towards their ultimate goals and aspirations.

Job responsibilities
Daily assessment of any new referrals for Tukes support services
Completing 4 weekly reviews either face to face or telephone
Accurate input into spreadsheets and S1 to ensure information around member numbers are current and relevant
Ensure response to calls are recorded and responded to within the given time scales
Liaise with internal teams to promote Meaningful Use of Occupational Time as part of service users care/ development pathway
Ability and flexibility to work in various environments and sites both indoors and outdoors.
Liaising with Leads within the service teams, Green and Blue prescribing team at Grimsby Garden centre, Tukes Navigator Support team including Peer mentors regarding individual cases once pathways are identified.
Attending internal and external meetings and webinars as a representative of the Tukes Team where required
Reporting outcome measures as evidence to assist in the development of the service needs.
Signpost support on Safeguarding if need identified.
Ensure you meet organisational training requirements.
Play an active role in member engagement within the services to ensure aspirations and wellbeing measures are met.
Promote One Front Door Hub activities to TUKES members
Promote internal Group and Activities
Promote Tukes Community membership Support adults who attend Tukes services for meaningful occupation of time
Promote and increase awareness of available social activities and events, and actively promote the service availability.
Providing information on how members can access local mental health services including crisis.
Co-produce, monitor and review recovery with the guidance of the Social prescribing navigator within and between services to ensure service users needs are met.
Promoting relationships with partners for Blue social prescribing e.g., Water Walking, Stand up Paddle boarding (SUP), Open water Swimming, Lakeside/river walks, Marshland trails, River cleaning.
Promoting relationships with partners for Green social prescribing and continuing the progress of existing ones e.g., Horticultural experiences at Grimsby Garden Centre, Walking with WHiSE and Forest foraging.
Encouraging Beneficiaries who will come from a range of groups who often experience mental health challenge, poor physical health, or social isolation, including but not limited to:
oUnemployed
oOlder people (including those with dementias)
oYoung people 16+
oBAME community
oAutism Awareness
oLBTG+ Community
oThose with addictions
