What to Expect (Job Responsibilities):
- Review monthly Care Gap data and complete virtual annual wellness visits for assigned centers
- Perform Transitional Care Management (TCM) non-face-to-face patient visits and oversee chronic care management
- Contact insurance Case Managers to follow up with patients and implement preventive care plans
- Analyze Healthy Planet Performance data to identify areas for improvement and communicate findings
- Maintain clerical aspects of medical records in accordance with established guidelines

What is Required (Qualifications):
- Minimum of 1-year experience in an ambulatory primary care setting
- High School Diploma or GED required
- Licensed Practical Nurse (LPN) certification required at time of hire
- Basic Life Support (BLS) certification required at time of hire
- Minimum of 5 years of relevant work experience preferred

How to Stand Out (Preferred Qualifications):
- 2 years of experience in an ambulatory primary care setting
- Associate's degree in a relevant field preferred
