- Satisfactory level of General Education - 5 GCSE Grade C/4 or above
- Good standard of written and spoken English - GCSE Grade C/4 or above
- NVQ3 or above in relevant subject
- CARE Certificate issued by UHL
- Must be able to demonstrate behavior's consistent with the Trust's Values and Behavior's
- Experience using computer systems eg. Microsoft Office/Sharepoint
- Experience in training others, passing on knowledge
- Minimum 1 year experience in a similar role
- Experience in using remote monitoring equipment/technology
- Communicates effectively with the public/patients and colleagues
- Ability to work as part of a team and act on own initiative
- Ability to receive and carry out instructions efficiently
- Ability to assist in teaching others and assess their understanding
- Experience in medical terminology and updating patient notes

Job summary
To support the Virtual Wards within our hospitals and the wider community through supporting patients to use technology and remote monitoring equipment in their own homes. You will visit patients in hospital, or at home, and teach them how to submit their observations for review by the clinical teams. You will be expected to liaise with Virtual Ward clinical colleagues to ensure appropriate patients are referred to the Virtual Ward and are managed effectively.
You will communicate with patients/carers to ensure a smooth transition onto the Virtual Ward and successful discharge from the Virtual Ward. After required training you will support the clinical teams by collecting samples from Virtual Ward patients, in hospital or at home, when necessary.
Main duties of the job
Provide clinical and non-clinical support to the multi-professional team under the direction of an appropriate registered health care professional.
Deliver direct and indirect care as delegated under the direction and supervision of an appropriate registered health care professional.
Support the delivery of effective health care to patients/service users by undertaking duties as identified by the multi- professional team.
Undertake specified and unsupervised aspects of care and therapeutic intervention as delegated by the registered health care professional within an agreed protocol, sphere of responsibility and after completingappropriate competency- based training agreed by the Trust.
Support the day to day management of the Virtual Ward equipment and stock for all pathways.

Assist the registered professional in developing care plans; assessing anddocumenting care and providing verbal and written reports in an effective and safe manner.
Make observations of care with regard to supporting the patients/users general health and wellbeing.
Maintain effective communication with the Registered Nurse/health care professional/Virtual Ward Team Leader regarding issues relating to assigned duties, promptly reporting areas of concern.
Assist and support care delivery by only undertaking delegated work that is within own level skill and competence.
Carry out duties consistent with individual patients/users wishes, maintaining privacy, dignity and recognising cultural diversity and needs.
Contribute to the overall effectiveness of the care delivered by the health care team and which benefits the patient/user experience.
Assist and contribute to the efficient use of stock, sundries and resources within Virtual Wards.
Take responsibility for maintaining own and others Health, Safety and Security.
Act in a professional manner and adhere to the UHL uniform policy.
There will be a requirement to visit patients at home therefore a UK Driving License is required for this post. The Trust will cover business mileage expenses if the vehicle is insured for Business Use.
You may be required to work cross-site, this includes the three main hospital sites at GGH, LGH and LRI.
