What to Expect (Job Responsibilities):
- Provide one-on-one nursing and personal care to clients
- Ensure standards of hygiene, nutrition, and homemaking are met and maintained
- Collaborate with the interdisciplinary home care team to optimize clients' health
- Follow a plan of care with oversight from a Registered Nurse or Clinical Team Manager
- Maintain accurate documentation of care provided

What is Required (Qualifications):
- Must hold a Vermont LNA license
- Minimum of one year of experience, preferably in a home health setting
- Valid Vermont driver's license
- Reliable vehicle with automobile insurance
- Ability to work weekends (Saturday and Sunday)
