      Introduction to the role of the team:
      We are recruiting to a position in the Yeadon Neighbourhood Team.
      Your role:

      The role will involve working with a wide range of customers and their carers in a strengths based way, helping people to live as independently as possible for as long as possible. You will be responsible for assessing and working with people to devise a support package that reflects the assessed needs of the individual. It will also involve signposting, linking with the 3rd sector as well as giving advice and assessing what level of support is needed. You will also have the opportunity to work with professionals from other agencies in the delivery of services.

      Front line:

      You will have excellent communication skills and be a team player. You will be motivated and a critical thinker with the ability to understand the needs of your customers.

      What you will bring to the role:

      The ideal candidates will possess demonstrable literacy and numeracy skills and will have the ability to plan, evaluate and prioritise work. We are looking for candidates with excellent interpersonal and communication skills.
      A knowledge and understanding of health and social care issues as well as the ability to work within Leeds City Council policy and procedures is essential.

      A Disclosure and Barring Service (DBS) check against the Adults barred list will be carried out on preferred candidates.
