What to Expect (Job Responsibilities):
- Provide quality and cost-effective management of a caseload of patients with complex wound care needs
- Assess, formulate, and execute plans of care using image-based remote patient monitoring
- Coordinate referrals, prescriptions, and durable medical equipment to drive positive outcomes
- Consult with home health agencies, primary care clinicians, and wound care clinics to provide education and guidance on wound care
- Participate in quality management/improvement activities and continuous quality improvement projects

What is Required (Qualifications):
- Nursing Degree
- Minimum four (4) years of wound, ostomy, and continence nursing (WOCN) or Nurse Practitioner (NP) experience
- Minimum two (2) years of wound care experience
- Licensure and credentials in Colorado, with the capability to be licensed and credentialed in multiple states in the future
- National Provider Identifier (NPI) and WOCN certification
- Current evidence-based knowledge of wound nursing practice
- Experience with wound assessments and various debridement techniques
- Strong communication and problem-solving skills
- Ability to work in interdisciplinary teams and provide patient education
- Proficiency in operating personal computers and technology platforms

How to Stand Out (Preferred Qualifications):
- Experience working with telehealth platforms
- Familiarity with virtual conferencing and remote image support
