Not afraid of a challenge? We are looking for enthusiastic and energetic Family Support /Transition Workers to work in our Transition Team which is based in Central Croydon and is a dynamic and vibrant service for young people aged between 17 and 25.
Although we sit in Adult Social Care, our journey with the people we support starts much earlier with the aspiration to start at academic year 9. Within Croydon Council, the Transition Team sits in a favourable position of being able to undertake work across both Children and Adult Services.

For this role you will need to have experience of working with young people with disability and/or other presenting needs which may at times be complex. We need committed knowledgeable workers to help us with our core business – empowering and supporting people who need signposting, community support, or even some direct help from the Council. We have a strong operating model of strength-based practice.
At this difficult time the Council Leadership has made clear their support for our statutory functions, including Adult and Children’s Social Care. We know where our priorities are.
It is expected that Family Support Workers will provide impact / efficacy of their intervention, be exemplary in engagement and relationship building with young adults and families, have the ability to evidence multi-agency partnership working and make use of critical reasoning modelling and facilitating reflective and evidence informed practice.
We work with young people who have a wide range of additional needs and complex conditions. We challenge inequality and traditional responses to service delivery and ensure that we employ a strength-based approach to our work.
The responsibilities will include joint working with Children’s Services and Special Educational Needs (SEN) , developing Transition support plans for young people, undertaking reviews, completing carers’ and adult social care assessments, attending LAC ( Looked After Child ) review meetings, carrying out Safeguarding initial information gathering and triaging under the supervision of your supervisor and attending relevant meetings with partner agencies.

The Opportunities
In the context of developing our services to continue to meet the challenges of the current situation we have a clear emphasis on Strength Based working, including short- and longer-term work. You will have the opportunity to have training from Children and Adult directorates. You will receive a comprehensive induction, regular supervision from a named supervisor, practice learning sessions, appraisal and annual salary increment. Your case load will be based on case weighting.
